Description:

Understanding of clinical services provided within these areas and able to discuss needs of service users within these areas and gather information required.

Gather clinical information relating to service users placed out of area daily.

Effective working relationships will be developed with all clinical services within the organisation.

To participate within the clinical Improvement Forums and Operational Management Meetings.

Chair meetings related to capacity and flow management.

Develop and support the interface and relations between ward teams, urgent care teams and community teams.

Develop and support the interface and relations between the rehabilitation service and external stakeholders; including the Accommodation Gateway, independent providers, landlords and Adult Social Care.
