Typical day as a Lead Support Worker (known by us as Coordinator)
 

  • Creating support plans with your customers and Working closely with Social workers, CPN/ community treatment team, probation and North Tyneside Recovery partnership to ensure we have the best wrap around support for the customer.
  • We help our customers with a little bit of everything. This includes enabling out into the community, prompting with medication, supporting with their finances, helping with their benefits and much, much more!
  • Customer group - We support our brilliant customers with their mental health, learning disabilities, substance misuse, forensic history and dual diagnosis.
  • Our customers are all under 117 aftercare which is to help customers who have been sectioned under section 3, 37,45A, 47 or 48 of the Mental Health Act 1983 at any time in their lives live independently in the community.
  • Supporting customers with daily living activities such as, shopping, budgeting, maintaining their tenancy and moving onto independent accommodation if they are able.
  • Carrying out risk assessments, support planning, goal setting and regular reviews.
  • Cuppas, chats and catch ups. There’s no rushing from one customer to the next here! Plenty of time to collaborate with colleagues too.
