An exciting opportunity has arisen to join the nurse led community heart failure service delivering care to the adult population of South East Essex.
The team based in Leigh on Sea is seeking a registered nurse to support the Clinical Nurse Specialists to deliver care and treatment in the primary care setting to patients with a confirmed diagnosis of heart failure.
The successful applicant will have experience within an acute cardiology setting and have a significant understanding of cardiac disease and management and an excellent understanding of heart failure management. They will have admirable communication and listening skills and conduct themselves professionally at all times. Work experience within the community setting would be an advantage.
They will be dynamic in their approach and be able to demonstrate abilities to adopt a problem solving approach to supporting and implementing the management plans for these complex patients whilst working independently.
The successful applicant will demonstrate an ability to work well within a team and have an understanding of the structures of primary and secondary care services and how the two organisations interface, particularly within the Long Term Conditions arena.
Main duties of the job
Whether working in a clinic or domiciliary setting the team’s vision is to deliver a sleek, modern evidence based service to all patients within settings close to or within their homes to promote independent self management of their chronic condition, consequently avoiding admission and readmission.
The successful applicant will be prepared to develop within the role and be responsible for achieving agreed objectives through an appraisal system within a negotiated timescale.
The Nurse led service continually endeavours to improve and develop service delivery and the post holder will be expected to support the progression of development plans.
Responsible for the Assessment, Planning and implementation of clinical management
Works in partnership with other groups in the development of innovative clinical practice.
Assists in managing a caseload, in line with CCG policies, guidelines and national standards.
Maintains a high professional profile and role model for nursing and health care services within Essex Partnership University Trust (EPUT).
Promotes and enables patient choice and independent living.
Assists in the prevention of unplanned hospital admissions, inappropriate A&E attendances and promotes and facilitates early hospital discharge where appropriate.
