You'll connect service users with essential resources, empowering them and promoting positive mental health outcomes. Typical day as a Community Navigator
 

  • Engage directly with our customers to develop personal, tailor-made support plans that reflect their aspirations and needs.
  • Lead and coordinate multidisciplinary teams during MARAC, safeguarding, and other multi-agency health meetings to ensure cohesive support strategies.
  • Advocate passionately for your customers, ensuring their access to necessary community resources and services that align with their goals.
  • Monitor and adapt support strategies, ensuring they remain aligned with individual goals and are responsive to changing needs.
  • Use your strong influencing and negotiation skills to guide customers towards making beneficial decisions for their mental health and wellbeing.
  • Maintain meticulous records of customers’ progress, applying agreed screening processes for eligibility and referral with precision and care.

You bring
 

  • A deep understanding of and empathy for individuals’ life stages and challenges, paired with an exceptional ability to listen and adapt to diverse needs.
  • Proven experience in creating and implementing effective, person-centred support plans.
  • Robust coordination and assessment skills, capable of managing referrals and ensuring seamless service delivery.
  • Strong interpersonal skills to work effectively within a team and lead by example in mentoring and sharing knowledge.
  • An eye for detail with a commitment to working within legislative frameworks and organisational policies.
  • A resilient nature, capable of working independently in dynamic environments, and an unwavering passion for supporting others to live their best lives.
